When the findings will not affect the treatment choices, the test is not reasonable or necessary. Certain uses of lumbar MRI are considered investigational and are therefore not covered by Medicare. I suspect (but do not know) that Medicare probably covers MRIs and MRI/TRUS-guided biopsies for men who have a rising PSA and a prior negative biopsy.
